However if you are banned, most of the times you will still get back in the index after 3 or 4 weeks if you fix the problems that you were banned for. Google is good in that!

If you are not appearing in the search engine even with the website address then make sure that you are resting in their sandbox,...

You will see that your listing is dropped in the particular search engine.

The word "penalized" may be a bit of a misnomer. You may have some of your links "discounted" because of the way you have set them up. For example:

Let's say you purchased "paid links" from a specific web site and the search engine discovered this site was selling "paid" links. It does not really "penalize" your site, it simply marks all links from that source as of "no value". This would cause your web site to fall in the rankings and until you replace those links with new "unpaid" links your site will not climb back up in the rankings again. There will be no easy way for you to know if/when your link(s) were penalized or even which ones were penalized. It would be better to simply avoid the practices that are penalized by the search engines.

There are many reasons why a site may fall in the rankings, many of which could be out of your control. Following good linking practices will help you avoid most problems.

Search engines won't send you any warnings and in fact are notorious for not telling you anything about what is wrong (especially Yahoo). Google Webmaster Tools is the best I've seen. They will work with you to some extent in letting you know what is wrong with your site but I've never seen a site get a warning before getting a drop in rankings for going outside the webmaster guidelines.

Google does not penalize sites if you're doing something wrong. If you're trying to sneak around and have a link farm, somewhere, Google frowns heavily upon those.
