I believe you can notify about this change to google at the webmaster tools, I just hope that your back links are created to "ursite.com" and not to "ursite.com.index.asp" otherwise you will lose all your back links effort. Once you notify google about this change your new file will started getting indexed again.

This is okay as long as you declared it on your .htaccess file.

Google "site:yourdomain.com" without the quotes and see if the new page has been indexed. I'd also recommend using a 301 redirect to get traffic that is trying to get to your old page to your new one.

Make sure you do a 301 redirect from your .asp index page to your .htm index page. As someone mentioned earlier, also make sure your Google Webmaster Tools knows about this change.

Absolutely, have 301 redirection in place and everything will be alright. Otherwise, search engines would consider the .htm page as a new one and a copy of the old .asp page.
