Hello, i have 2 questions regarding Virginia Sales Contract

1)If only I am going to be on the mortgage but me and my fiance want to be on the deed, do both of us need to be in the sales contract? if not when and who do we notify so everything is in order at settlement?

2)Besides the sales contract what other forms should i be signing that go with the sales contract when we are about to submit our first offer?
