Good Example of Quality Page Optimization
I do better with looking at the picture, as opposed to reading about it. In other words, I'm visual. Can anyone recommend a page or two that is a good example of on page SEO. Especially in regards to keyword density, keyword placement, and the use of <h> & BOLD tags.

Truthfully, you can't break it down that easy. SEO is not only comprised of on-page constructs, but also site wide themes, hierarchy of pages, and off page factors. It is the relationship and interplay between these factors that SEO's try to balance in order to achieve better rankings.

If x > 20% then goto ABCD
and so on and so on.
With that being said, here's how you should write your web page:
1. Headlines should be used to grab the attention of the web surfer and what interests them. If you use an H1, make it the first headline on the page. The next should be H2, and the next H2 or H3.
2. Write your copy for the page. Then read it out loud. Does it sound natural, or does it sound like a keyword stuff piece of fluff? Make adjustments. That will help you with your KW density.
3. Bold and strong tags don't carry too much weight in SEO anymore.
4. Make sure that your title tags are relevant to the primary topic of the page.
5. Make sure that your site has a well defined, thematic navigation structure.
I know this is probably not the nitty gritty details you are looking for, but the truth is that you can no longer define SEO as optimizing content on a single page.
There are too many other factors involved, especially for Google. You can no longer optimize a page and throw a bunch of links with specific anchor text at the page and shoot to the top of the rankings.

Do you have a resource to read further on what you have discussed in the post? What you've stated is no surprise to me, as seo is going the direction of 'natural.' The search engines are becoming adept to picking up any form of gray hat seo (black hat has gone by the wayside).
